Registration Notice
1.The registration will be opened starting on the 13th July 2010. The
participants are allowed to register starting on that day.
2.This program is open to ALL PERUBATAN`s members including dentistry
and pharmacy students.
3.The participants can register using two methods. These methods are:
a) Registration using the online form
i) This method of registration is compulsory.
b) Registration using the text message system (SMS).
i) This method is exclusive for the participants who do not have
any access to the internet.
Therefore, the participants can register his/her names via SMS
to our hotline numbers
ii) The participants need to send his/her details using the guideline
that will be given.
iii) [FULL NAME], [Years of study], [University], [State in Malaysia]
Eg. MUHAMMAD HAFIZ Bin MANSOR, 2nd Year, Zagazig University,
Perlis
iv) After sending the SMS to the given number, it is compulsory for
the participants to get to the blog to submit details using the
online form.
v) The SMS method is for the participant to book their names for this
program.
4.Guideline to register using the online registration form.
a) The participants need to answer all the questions inside the form.
b) The participants need to answer according to the given format for
some questions.
c) If some participants have an allergy, please state it on the allergy
questions.This is very important to make sure that our squad
members provide special care to these participants.
d) The participants can choose either they want to use their own
transportation or using the provided transportation to go to the
Nur Lembah Pangsun. If the participants need the transportation,
check “yes” for the transportation question. The participants are
allowed to use their own transportation.
5.Any change of participant with other or wants to cancel his/her name can be done before 19th July 2010. After this date, any change or cancellation are not allowed.
6. This program is limited to a certain number of the participants. So be the first person to register the name. The name of the participants that had been registered and accepted to join this program will be announced later.
7. Any appeal after names have been finalized will not be entertained.
